What insurance/payment do you accept?

  • We accept self-pay
  • We accept the following insurance: Dean Health Plan. Group Health Cooperative (GHC) of South-Central Wisconsin, Comprehensive Community Services (CCS), Medicaid (BadgerCare), Medicare, Aetna, and United Health Care.

Who do we serve?

  • General Public:
    • Anyone, ages 13 and up, needing mental health prescribing services.
  • Comprehensive Community Services (CCS):
    • Prescribing and other services. Please call to check.

How do I make an appointment?

Note: Current CCS clients should contact their CCS Service Facilitator for paperwork and initial intake setup.

Do you provide therapy?

  • We currently do not offer psychotherapy but plan to in the future. See our full services list here.

What are your office hours?

  • Open Monday to Thursday, 9:00 am – 4:30 pm. See additional contact information here.

How long are appointments?

  • Initial appointments: Approximately 60 to 90 minutes.
  • Follow-up appointments: Around 30 minutes.

What are the client’s responsibilities?

  • You are responsible for fees that are not covered by your insurance, when applicable.
  • You agree to arrive on time and conduct yourself in a respectful manner during appointments, and to attend all scheduled appointments.
  • You agree not to arrive at the office under the influence of any substance. If you do, you will not be seen.
  • You agree that you will take medication as ordered and not self-adjust.
  • You agree that the medication you receive is your responsibility and that you will keep it in a safe, secure place. You agree that lost medication will not be replaced regardless of the reasons for such loss.
  • You agree not to seek similar medications from any physicians, pharmacies, or other sources without informing your treating physician.
  • You understand that not using medications as prescribed/or mixing medications with drugs of abuse, can be dangerous.
